Your first visit is an opportunity for our clinicians to learn about your history, current symptoms, treatment goals, and any medications you may be taking. This may involve:

  • Collaboratively setting next steps for your care plan

What to Expect After Your Initial Visit

Required Intake Forms

All forms are hosted digitally in Healthie. For in-person appointments, paper backups are available.


When Forms Need to Be Completed

Forms must be completed before your first appointment. Reminder emails are sent at 72 and 24 hours before your visit. Our staff may also call to assist if needed.

Crisis Support

If you are experiencing a mental health emergency or are in immediate danger:

  • Call or text 988 (Suicide & Crisis Lifeline)
  • Call 911 for emergencies

Mygda does not provide emergency or crisis services.
